Release of Information
If you wish to release information regarding your care to any individual or agency, please complete and sign Release of Information form. A completed Release of Information is required for each individual or agency you would like to receive information.
MEDICARE PRIVATE CONTRACT
Our clinic does not file with Medicare. If you are choosing to be seen at our office as a self-pay patient, please fill out this Medicare Private Contract acknowledging the fact that you are aware we do not accept your insurance and you are foregoing your right to file.
